The City of Berkeley and Carbon Health are holding a Vaccination Clinic at the Tarea Hall Pittman South Branch and Tool Lending Library. The clinic will be held in the Tool Library Parking Area.

This City-coordinated clinic is for ages 5+ provide the following vaccine and doses:
Pfizer vaccine: Standard (12+) and Pediatric doses (ages 5-11)
Pfizer doses 1 & 2, additional dose (immunocompromised), and boosters
Make an appointment with Carbon Health or walk-up.
If no appointments appear on the clinic link, appointments are fully booked.
Limited walk-up appointments available, subject to staffing and supply.
Parental Consent: Ages 5-17 require parent/guardian consent: in-person, by phone, or via signed form (see sample). Ages 5-15 must be accompanied by a responsible adult before, during, and after appointment.
